BOTOX® Maintenance: How Often Should Someone Get Injections?

BOTOX® is a perennially popular treatment with patients who want a refreshed, younger-looking appearance, but who are not considering surgery, such as a facelift. The nonsurgical process is known to be virtually pain-free (injections feel more like pinches than anything), and patients often go back to their day-to-day activities right away. Board-certified plastic surgeon Dr. David Hartman, who has the distinction of being an ExpertInjector™ (trained and properly qualified to administer injectables and adhere to very strict patient safety standards) regularly receives questions from patients who are curious about what maintenance for BOTOX® is like. The Ohio-based physician explains that when it comes to BOTOX®, it is critical to consult a qualified provider who will help develop a long-term strategy that fits a patient’s individual needs.
Before delving into the finer details of BOTOX® maintenance, it’s helpful to explain more about the prescription cosmetic and how it affects the body. BOTOX® is the brand name of a particular neurotoxin (a substance that modifies the function of the nervous system) called botulinum toxin type A, which is derived from the organism known as c. botulinum. The active ingredient is diluted in saline. The miniscule amounts of the neurotoxin contained in the drug cause muscles to temporarily relax in the affected areas of the face by interfering with the typical transmission of signals. Reducing the activity of certain muscles is a very effective way to keep age-related lines and creases from appearing.
BOTOX® is FDA-cleared for use around the eyes, on the forehead, and between the brows, but it also has many safe, off-label cosmetic uses (treating “bunny lines” on the nose, for instance), as well as FDA-approved therapeutic applications, such as providing relief for migraines and muscle spasm disorders.
People often get their first cosmetic BOTOX® injections in their 30s, as this is usually when signs of aging start to become especially noticeable. During the procedure itself, a topical numbing agent or ice may be applied to minimize discomfort for patients—especially those who have more sensitive skin—but the small needle used generally causes only a slight pinprick sensation. Some patients experience minor swelling or light bruising related to the injection.
Once injected, BOTOX® works to reduce the appearance of unwanted wrinkles within a few days. But how many treatments are necessary, and how will patients know when to go back for more injections? The answers vary, depending on an individual’s cosmetic goals, patient-physician preference, the dosage used (injecting a larger amount can prolong the effects), and the treatment area.
It’s important to note that while BOTOX® is generally long-lasting (four to six months on average), it doesn’t permanently correct wrinkles. Many patients find that upkeep requires multiple sessions over the course of the year as it gradually wears off. It’s recommended that anyone who wants to maintain a more youthful look with BOTOX® schedule new sessions after the muscles in their face have regained some motion. Each patient’s needs and goals for the cosmetic treatment are unique and are often influenced by age and the severity of facial creases. Injectors should work directly with each patient to develop a personalized BOTOX® plan.
Patients typically start to see the initial results of their injections within a few days, but it may take three to four weeks for the full effects of BOTOX® to be seen. There are ways patients can maximize their results. Following any post-treatment instructions from the physician, such as avoiding exposure to direct sunlight and alcohol consumption, can help generate the best possible outcome. Patients are also advised to maintain a healthy diet and exercise routine, as well as moisturize daily as part of a proper skincare regimen. Of course, scheduling procedures on a regular basis can also help to boost the effects of BOTOX®.
